To receive VA educational aid you must have the following documentation:

• Copy of DD Form-214
• The Application for VA Educational Benefits (VA Form22-1990)
• Request for Change of place of training (VA Form 22-1995)
• Dependents’ application for VA Education Benefits (VA Form 22-5490)
• Dependents’ request for change of Program ( VA From 22-5495)
• Transcripts from schools previously attended
